*Add the MP4 files of your videos to a Google Drive Folder and put the URL of the folder in the field above called “Shared Google Drive folder URL of the Videos”.
**The Statement of Originality can be downloaded here. After you download the PDF, it’s recommended that you open it in Google Docs, where you can fill out the various fields. To add an electronic signature to the Doc, follow the instructions on this page. Then save the Google Doc as a PDF. This is the document that you will upload above.
***The Release Form can be downloaded here. Follow the same instructions as outlined for the Statement of Originality. There will need to be a separate release form for each interviewee, that’s four in total.
